Zusammenfassung: Mit einer Lebenszeitprävalenz von 44 % bei Erwachsenen im Alter von 55 Jahren oder älter sind Frakturen nach wie vor ein großes Problem der öffentlichen Gesundheit, wobei die Frakturwahrscheinlichkeit aufgrund von Osteoporose mit dem Alter stark zunimmt. Die distale Radiusfraktur ist die dritthäufigste Fraktur in Deutschland. Skaphoidfrakturen stellen die weitaus häufigsten Brüche der Handwurzelknochen dar und führen bei inadäquater Diagnostik und Therapie oft zu problematischen Heilungsverläufen. Eine sorgfältige Anamnese, körperliche Untersuchung und differenzierte Bildgebung helfen, Frakturen nicht zu übersehen, korrekt einzuschätzen und die bestmögliche Behandlung vorzunehmen und damit optimale Ergebnisse zu erzielen. In Abhängigkeit der Fraktur können konservative, wie auch operative Therapieverfahren indiziert sein. Insbesondere bei der konservativen Therapie sind regelmäßige und engmaschige Röntgenkontrollen erforderlich.

Summary: With a lifetime prevalence of 44% in adults aged 55 or older, fractures remain a major public health problem, with the likelihood of fracture due to osteoporosis increasing significantly with age. Distal radius fractures are the third most common type of fracture in Germany. Scaphoid fractures are by far the most common type of carpal bone fracture and often lead to problematic healing if not diagnosed and treated properly. A careful medical history, physical examination, and differentiated imaging help to ensure that fractures are not overlooked, are correctly assessed, and are treated in the best possible way, thus achieving optimal results. Depending on the fracture, conservative or surgical treatment may be indicated. Regular and close X-ray monitoring is particularly important in conservative treatment.

Zusammenfassung: Das knöcherne Grundgerüst des Daumensattelgelenkes bilden die Basen des OsmetacarpaleI sowie des Ostrapeziums. Die Stabilität dieses Gelenks beruht auf dem feinen Gleichgewicht zwischen den passiven Bänderstrukturen. Die Rhizarthrose stellt mit einer Prävalenz von 7% für Männer und 15% für Frauen eine der häufigsten Arthrosen des Körpers und eines der häufigsten Krankheitsbilder der Handchirurgie dar. Bewegungseinschränkungen des End- oder Grundgelenkes des Daumens beeinträchtigen die Handfunktion deutlich geringer als der Verlust der Oppositionsfähigkeit bei Rhizarthrose. Insbesondere im Anfangsstadium kann eine konservative Therapie erfolgreich durchgeführt werden. Der Goldstandard der operativen Behandlung in Deutschland stellt die Resektions-(Interpositions)-Arthroplastik dar, wobei der endoprothetische Ersatz zunehmend an Bedeutung gewinnt. Die Entscheidung, welche Therapie durchgeführt wird, muss im Rahmen eines gemeinsamen Entscheidungsprozesses individuell getroffen werden, wobei das Alter, das Aktivitätsniveau und die Risikotoleranz der Patientin bzw. des Patienten sorgfältig gegen die jeweiligen Vor- und Nachteile der einzelnen Ansätze abgewogen werden müssen.

Summary: The bony framework of the thumb saddle joint is formed by the bases of the first metacarpal bone and the trapezium bone. The stability of this joint is based on the delicate balance between the passive ligament structures. With a prevalence of 7% in men and 15% in women, rhizarthrosis is one of the most common forms of osteoarthritis in the body and one of the most common conditions treated in hand surgery. Restricted movement of the end or base joint of the thumb impairs hand function significantly less than the loss of opposition in rhizarthrosis. Conservative therapy can be successful, especially in the early stages. The gold standard of surgical treatment in Germany is resection (interposition) arthroplasty, although endoprosthetic replacement is becoming increasingly important. The choice of therapy should be made individually as part of a joint decision-making process, carefully weighing the patient‘s age, activity level, and risk tolerance against the respective advantages and disadvantages of the individual approaches.

Zusammenfassung:Der Morbus Dupuytren hat weltweit eine Prävalenz von etwa 8%. Die Veränderungen der Palmaraponeurose wurden klinisch erstmals 1614 von Felix Platter in Basel beschrieben. Beim Morbus Dupuytren bilden sich durch Umwandlung (Metaplasie) von Fibroblasten in Myofibroblasten aus diesen Faszienstrukturen zunächst Knoten und später Stränge. Die Proliferation der Myofibroblasten führt zu einer Verkürzung der Dupuytren-Stränge, wodurch Kontraktionskräfte auf das umgebende Gewebe wirken. Diese Proliferation wird durch verschiedene Zytokine wie z.B. TGF-?, PDGF und IL-1 vermittelt. Innerhalb der Myofibroblasten interagiert ?-Aktin mit dem extrazellulären Fibronektinkomplex. Genetische Einflüsse erhöhen die Prädisposition, aber verursachen nicht allein die Erkrankung. Daneben existieren weitere Risikofaktoren, bspw. körperliche Arbeit, vorausgegangene Handverletzungen, Diabetes mellitus, Rauchen und Alkoholkonsum. Das Ausmaß der Kontrakturen wird mithilfe der Tubiana-Klassifikation eingeschätzt. Die konservative Therapie umfasst Infiltrationen, Handtherapie, Radiatio bzw. Stoßwellentherapie. Minimalinvasive Therapien beinhalten Injektion mit Kollagenase oder die perkutane Nadelaponeurotomie. Die am häufigsten angewendete und als Goldstandard geltende chirurgische Therapie ist die partielle Aponeurektomie.

Summary: Dupuytren‘s disease has a prevalence of approximately 8% worldwide. The changes in the palmar aponeurosis were first described clinically in 1614 by Felix Platter in Basel. In Dupuytren‘s disease, the transformation (metaplasia) of fibroblasts into myofibroblasts initially forms nodules and later strands in these fascial structures. The proliferation of myofibroblasts leads to a shortening of the Dupuytren‘s cords, causing contractile forces to act on the surrounding tissue. This proliferation is mediated by various cytokines such as TGF-?, PDGF, and IL-1. Within the myofibroblasts, ?-actin interacts with the extracellular fibronectin complex. Genetic influences increase predisposition, but do not independently cause the disease. There are also other risk factors, such as physical labor, previous hand injuries, diabetes mellitus, smoking, and alcohol consumption. The extent of contractures is assessed using the Tubiana classification. Conservative therapy includes infiltrations, hand therapy, radiation, and shock wave therapy. Minimally invasive therapies include collagenase injection or percutaneous needle aponeurotomy. The most commonly used surgical therapy, considered the gold standard, is partial aponeurectomy.

Zusammenfassung: Patellofemorale Beschwerden und Instabilitäten stellen ein häufiges und klinisch relevantes Krankheitsbild dar. Die Ursache ist in der Regel multifaktoriell und umfasst knöcherne Dysplasien, ligamentäre Insuffizienzen sowie Achs- und Rotationsfehlstellungen. Eine isolierte Betrachtung einzelner Befunde führt nicht selten zu unzureichenden Therapieergebnissen.Ziel dieses Beitrags ist die Darstellung einer strukturierten klinischen und bildgebenden Diagnostik des Femoropatellargelenks mit besonderem Fokus auf die operative Entscheidungsfindung. Dargestellt werden die wesentlichen Elemente der klinischen Untersuchung sowie die Rolle der konventionellen Radiologie, der Magnetresonanztomografie und der Torsionsdiagnostik. Die diagnostischen Befunde werden im Kontext ihrer operativen Relevanz bewertet.Die klinische Untersuchung erlaubt eine erste funktionelle Einordnung der Instabilität. Die bildgebende Diagnostik dient der Identifikation prädisponierender Faktoren wie Trochleadysplasie, MPFL-Insuffizienz, Patellahochstand (Patella alta), Achs- und Tuberositasfehlstellungen. Erst die integrative Bewertung aller Parameter ermöglicht eine individuell angepasste operative Strategie.Die Diagnostik patellofemoraler Instabilitäten muss ursachenorientiert und strategiegeleitet erfolgen. Isolierte operative Maßnahmen ohne vollständige Analyse der zugrunde liegenden Pathoanatomie erhöhen das Risiko für Rezidive und persistierende Beschwerden.

Summary: Patellofemoral pain and instability are common clinical problems with a predominantly multifactorial etiology. Bony dysplasia, ligamentous insufficiency, and axial or rotational malalignment often coexist and contribute to instability. This article outlines a structured clinical and imaging-based diagnostic approach to patellofemoral disorders with particular emphasis on surgical decision making. Key elements of clinical examination and the role of conventional radiography, magnetic resonance imaging, and torsional assessment are reviewed.Clinical examination provides an initial functional assessment, whereas imaging identifies predisposing anatomical factors such as trochlear dysplasia, MPFL insufficiency, patella alta, axial malalignment, and tibial tuberosity malposition. Only a comprehensive and integrative evaluation allows an individualized surgical strategy.Diagnostic assessment of patellofemoral instability should be pathology-driven and strategy-oriented. Isolated surgical procedures without comprehensive analysis of underlying anatomical factors are associated with inferior clinical outcomes.

Zusammenfassung: Die isolierte patellofemorale Arthrose stellt in der Knieendoprothetik weiterhin eine besondere Herausforderung dar. Trotz moderner Implantatkonzepte bleiben die Ergebnisse der patellofemoralen Prothese heterogen und erreichen hinsichtlich Patientenzufriedenheit und Langzeitbeständigkeit häufig nicht das Niveau der Knietotalendoprothese. Ursache ist nicht nur die ausgeprägte Heterogenität des Patientenkollektivs, sondern vor allem die biomechanische Komplexität des patellofemoralen Gelenks. Zudem entsteht die patellofemorale Arthrose häufig nicht primär degenerativ, sondern als Folge von Fehlmechanik, Maltracking oder Instabilität.Gerade diese Konstellation macht die Therapieentscheidung besonders anspruchsvoll. Die isolierte patellofemorale Arthrose betrifft nicht selten jüngere, funktionell aktive Patientinnen und Patienten, bei denen die Knietotalendoprothese zwar ein verlässliches Verfahren darstellt, der Ersatz klinisch unauffälliger tibiofemoraler Kompartimente jedoch kritisch abgewogen werden muss. In dieser Situation kann die patellofemorale Prothese als kompartimenterhaltende Behandlungsoption funktionelle Vorteile bieten. Voraussetzung sind eine präzise Diagnostik, eine differenzierte Analyse der zugrundeliegenden Pathomechanik und ein realistisches Erwartungsmanagement.Der wesentliche Fortschritt der vergangenen Jahre liegt daher weniger in einer generellen Ausweitung der Indikation als in einem besseren Verständnis günstiger und ungünstiger Ausgangskonstellationen. Entscheidende Voraussetzungen für ein gutes Ergebnis sind der sichere Ausschluss einer klinisch relevanten tibiofemoralen Begleitarthrose, die Identifikation der zugrundeliegenden pathoanatomischen Ursachen, die frühzeitige Planung möglicher Begleiteingriffe sowie eine technisch präzise operative Umsetzung.Die patellofemorale Prothese ist damit keine Standardversorgung der isolierten patellofemoralen Arthrose, sondern ein selektives, biomechanisch anspruchsvolles Behandlungskonzept für klar definierte Patientinnen und Patienten. Bei korrekter Indikationsstellung kann sie sehr gute funktionelle Ergebnisse erzielen und insbesondere bei jüngeren, aktiven Betroffenen einen totalendoprothetischen Ersatz sinnvoll hinauszögern oder sogar verhindern.

Summary: Isolated patellofemoral osteoarthritis remains a particular challenge in knee arthroplasty. Despite modern implant concepts, outcomes after patellofemoral arthroplasty remain heterogeneous and often do not match those of total knee arthroplasty in terms of patient satisfaction and long-term durability. This is attributable not only to the marked heterogeneity of the patient population, but above all to the biomechanical complexity of the patellofemoral joint. In addition, patellofemoral osteoarthritis is often not primarily degenerative, but rather develops as a consequence of malalignment, maltracking, or instability.This constellation is precisely what makes treatment decision-making demanding. Isolated patellofemoral osteoarthritis often affects younger, functionally active patients in whom total knee arthroplasty represents a reliable procedure, yet replacement of clinically unaffected tibiofemoral compartments must be weighed carefully. In this setting, patellofemoral arthroplasty may offer functional advantages as a compartment-preserving treatment option. However, this requires careful diagnostic work-up, a differentiated analysis of the underlying pathomechanics, and realistic expectation management.The major advance of recent years therefore lies less in a general expansion of indications than in a better understanding of favourable and unfavourable preoperative constellations. Key prerequisites for a good outcome are the reliable exclusion of clinically relevant concomitant tibiofemoral osteoarthritis, identification of the underlying pathoanatomical causes, early planning of potential concomitant procedures, and technically precise surgical execution.Patellofemoral arthroplasty should therefore not be regarded as a standard treatment for isolated patellofemoral osteoarthritis, but rather as a selective and biomechanically demanding treatment strategy for clearly defined patients. With appropriate patient selection, it can achieve very good functional outcomes and, particularly in younger and active patients, may meaningfully delay the need for total knee arthroplasty or even prevent it.

Zusammenfassung:Die vertiefende Trochleaplastik stellt ein Standardverfahren in der Behandlung der patellofemoralen Instabilität mit guten klinischen Ergebnissen dar. Offene Wachstumsfugen am distalen Femur mit der potenziellen Gefahr des Auftretens von Wachstumsstörungen durch Verletzung der ventralen Fugenanteile limitieren den Einsatz im Kindes- und Jugendalter. Bei sorgfältiger präoperativer Evaluation kann jedoch bereits bei Jugendlichen mit noch offenen Wachstumsfugen nach erfolgtem pubertärem Wachstumsschub die operative Korrektur mit gutem Erfolg sicher durchgeführt werden. Gleichzeitig stellen höhergradige Knorpelschäden an der Trochlea femoris eine technische Herausforderung dar und werden deshalb häufig als Kontraindikation gesehen. Liegen bereits fortgeschrittene Knorpelläsionen vor, muss die Indikation zur Trochleaplastik kritisch geprüft werden, aber auch in diesen Fällen sind noch gute klinische Ergebnisse möglich.

Abstract: Trochlear dysplasia is the most important risk factor for patellofemoral instability. Deepening trochleoplasty is a surgical procedure, which is able to reshape the dysplastic femoral trochlea in patients with patellofemoral instability. The ideal indication for trochleoplasty is documented patellofemoral instability and high-grade trochlear dysplasia. For adolescent patients with open distal femoral physis trochleoplasty can be safely performed up to 2 years before the projected end of growth with good clinical results without redislocation and with no growth disturbance. In addition even in the presence of degenerative changes of the patellofemoral joint for selected patients trochleoplasty shows acceptable clinical results. In conclusion neither open physes nor the presence of degenerative changes are strict contraindications.

Zusammenfassung: Die Trochleadysplasie ist bei patellofemoralen Pathologien mit Instabilitätsbeschwerden und auch bei der isolierten Retropatellararthrose überaus häufig zu finden und wesentlich für die Pathogenese. Eine genaue klinische und bildgebende Abklärung der oft heterogenen bildgebenden und klinischen Befunde ist wichtig, um eine geeignete Therapie zu empfehlen. Bei richtiger Indikation und einer effizienten Korrektur des Patella-Fehlgleitens zeigen sowohl die kombinierte Trochlea- und MPFL-Plastik als auch der retropatellare Gelenkersatz eine hohe Patientenzufriedenheit mit guten klinischen und funktionellen Ergebnissen. Der Schlüssel des Erfolges liegt in einer effizienten knöchernen Korrektur, der Beachtung des Weichteilalignements sowie in einer konsequenten frühfunktionellen Nachbehandlung.

Summary: Trochlear dysplasia is highly prevalent in patellofemoral pathologies associated with instability as well as in isolated patellofemoral osteoarthritis, and it plays a key role in their pathogenesis. A thorough clinical and imaging-based assessment of the often heterogeneous findings is essential to guide appropriate treatment. When the indication is appropriate and patellar maltracking is effectively addressed, both combined trochleoplasty with medial patellofemoral ligament (MPFL) reconstruction and patellofemoral arthroplasty can result in high patient satisfaction with favorable clinical and functional outcomes. The key to achieving these outcomes lies in precise bony correction, careful attention to soft tissue alignment, and a consistent early functional rehabilitation.

Zusammenfassung:Die laterale Patellaluxation gehört zu den häufigeren Verletzungen des Kniegelenks bei jungen und sportlich aktiven Patientinnen und Patienten. In mehr als 90% der Fälle kommt es zu einer Ruptur des medialen patellofemoralen Ligaments (MPFL). Dieses Band ist der wichtigste passive Stabilisator gegen eine laterale Verschiebung der Patella. Eine Luxationsverletzung und/oder anhaltende Instabilitäten der Kniescheibe können zu Knorpelschäden, osteochondralen Frakturen, wiederkehrender Instabilität, Schmerzen, Verlust der Sportfähigkeit sowie verminderter Alltagsaktivität führen und langfristig in eine patellofemorale Arthrose münden. Trochleadysplasien, andere knöcherne Fehlbildungen sowie Schäden der kniescheibenstabilisierenden Bandstrukturen stellen wichtige Ursachen dieser Problematik dar. Eine isolierte MPFL-Plastik ist daher nach umfassender Diagnostik nur in ausgewählten Fällen indiziert. Bei korrekter Indikationsstellung stellt sie jedoch ein wertvolles und vergleichsweise schonendes Standardverfahren dar.In den letzten Jahren wurden zahlreiche operative Techniken beschrieben. Diese unterscheiden sich vor allem hinsichtlich der Fixationsmethoden, der Transplantatwahl und Formation, der Umsetzung einer anatomischen Rekonstruktion sowie der intraoperativen Spannungseinstellung des Transplantats. Ziel dieses Beitrags ist es, unsere Technik der anatomischen MPFL-Rekonstruktion unter Verwendung von Hamstringsehnen oder Allografts vorzustellen. Diese Technik ist nicht nur als Einzelverfahren, sondern ebenso unkompliziert in Kombination mit anderen Verfahren, wie der Trochleaplastik, Gelenkersatzoperationen, Kreuzbandersatzplastiken, etc. durchführbar. Eine Besonderheit der Methode ist die implantatfreie Fixation einer doppelläufigen Transplantatschlinge. Die femorale Fixation erfolgt – neben einem langen Loop durch die Patella – über transkortikale Fäden. Ein zentrales Element der Technik ist die einfache intraoperative funktionelle Testung und Spannungsbalancierung vor der endgültigen Fixation des Transplantats. Neben einer Darstellung der konservativen und operativen Behandlungsmöglichkeiten wird unsere Operationstechnik detailliert beschrieben. Darüber hinaus werden die postoperative Rehabilitation sowie aktuelle wissenschaftliche Daten zur MPFL-Rekonstruktion und zu möglichen Vorteilen unserer Methode dargestellt.

Abstract: Lateral patellar dislocation is among the more common injuries of the knee joint in young and physically active patients. In more than 90% of cases, rupture of the medial patellofemoral ligament (MPFL) occurs. This ligament represents the most important passive stabilizer against lateral displacement of the patella. A dislocation injury and/or persistent instability of the patella may lead to cartilage damage, osteochondral fractures, recurrent instability, pain, loss of the ability to participate in sports, and reduced daily activity, and may ultimately result in patellofemoral osteoarthritis. Trochlear dysplasia, other bony abnormalities, and injuries of the patellar stabilizing ligamentous structures represent important causes of this condition. Therefore, isolated MPFL reconstruction is indicated only in selected cases after comprehensive diagnostic evaluation. When appropriately indicated, however, it represents a valuable and comparatively minimally invasive standard procedure.In recent years, numerous surgical techniques have been described. These techniques differ mainly with regard to fixation methods, graft choice and configuration, the implementation of anatomical reconstruction, and intraoperative graft tensioning. The aim of this article is to present our technique of anatomical MPFL reconstruction using hamstring tendons or allografts. This technique can be performed not only as a standalone procedure, but also easily in combination with other procedures such as trochleoplasty, joint replacement surgeries, cruciate ligament reconstruction, etc. A particular feature of this method is the implant-free fixation of a double-bundle graft loop. Femoral fixation is achieved—besides a long loop through the patella—using transcortical sutures. A central element of the technique is simple intraoperative functional testing and tension balancing prior to definitive graft fixation. In addition to outlining conservative and surgical treatment options, our operative technique is described in detail. Furthermore, postoperative rehabilitation protocols and current scientific data on MPFL reconstruction and the potential advantages of our method are presented.

Zusammenfassung: Die patellofemorale Instabilität stellt ein häufiges und zugleich komplexes Krankheitsbild dar. Primäres Ziel der Therapie ist das Verhindern weiterer Luxationsereignisse. Bei einer erstmaligen Luxation der Patella ohne relevante Begleitverletzungen und Risikofaktoren besitzt die konservative Therapie weiterhin einen relevanten Stellenwert. Jedoch ist eine zielgerichtete klinische und radiologische Diagnostik entscheidend, um vorliegende Risikofaktoren zu identifizieren und bestmöglich zu therapieren. Die operative Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) hat sich als Basistherapie etabliert. Bei Vorliegen relevanter anatomischer knöcherner Risikofaktoren wie einer Trochleadysplasie, einer Patella alta, einer pathologischen Lateralisation der Tuberositas tibiae sowie Torsions- und Achsfehlstellungen reicht eine isolierte MPFL-Plastik jedoch häufig nicht aus. In diesen Fällen sind knöcherne Korrekturen essenzieller Bestandteil eines erfolgreichen Therapiekonzepts.

Summary: Patellofemoral instability is a common yet complex clinical condition. The primary goal of treatment is to prevent recurrent dislocation events. Following a first-time patellar dislocation without relevant concomitant injuries or significant risk factors, conservative management continues to play an important role. However, focused clinical and radiological assessment is essential to identify underlying risk factors and to guide optimal treatment. Surgical reconstruction of the medial patellofemoral ligament (MPFL) has become the established cornerstone of surgical treatment. Nevertheless, in the presence of relevant osseous anatomical risk factors such as trochlear dysplasia, patella alta, pathological lateralization of the tibial tubercle, as well as torsional and axial malalignment, isolated MPFL reconstruction is often insufficient. In such cases, bony realignment procedures are an essential component of a successful treatment strategy.

Zusammenfassung: Die Schultersteife ist ein klinisches Syndrom mit Einschränkung der aktiven und passiven Beweglichkeit des Schultergelenks. Entscheidend ist die Unterscheidung zwischen primärer und sekundärer Schultersteife, da sich Ursachen, Verlauf und Therapie unterscheiden. Die primäre Schultersteife ist eine meist selbstlimitierende entzündlich-fibrotische Erkrankung der Gelenkkapsel mit einem typischen Verlauf aus Schmerz-, Steife- und Erholungsphase. Die sekundäre Schultersteife entsteht dagegen nach Trauma, Operationen, Frakturen, Rotatorenmanschettenrekonstruktionen, Schulterstabilisierungen, Kalktendinitis, etc. Neben kapsulären Kontrakturen bestehen häufig extrakapsuläre Adhäsionen im subakromialen, subdeltoidalen und subkorakoidalen Raum. Bei persistierender, funktionell relevanter Steife stellt die arthroskopische Arthrolyse ein erfolgreiches operatives Verfahren dar. Dabei werden intraartikulär die kontrakten Anteile der Gelenkkapsel einschließlich des Rotatorenintervalls sowie der anterioren, inferioren und gegebenenfalls posterioren Kapsel gelöst. Zusätzlich erfolgt bei sekundären Formen häufig eine extraartikuläre Arthrolyse mit Lösung subakromialer, subdeltoidaler und subkorakoidaler Verwachsungen, um die physiologische Gleitfähigkeit der Weichteile wiederherzustellen. Wichtig sind auch begleitende operative Verfahren, wie bspw. eine Manschettenrekonstruktion, Implantatentfernung, Kalkentfernung sowie die Versorgung von Bizepssehnenpathologien. Zahlreiche Studien zeigen nach arthroskopischer Arthrolyse sehr gute Verbesserungen von Schmerzen, Beweglichkeit und Schulterfunktion. Viele Patientinnen und Patienten erreichen wieder nahezu normale Alltagsfunktionen und können auch zu Überkopf- und Wurfsportarten zurückkehren. Eine konsequente Physiotherapie nach der arthroskopischen Arthrolyse ist entscheidend.

Summary: Shoulder stiffness is a clinical syndrome characterized by restricted active and passive range of motion of the shoulder joint. The distinction between primary and secondary shoulder stiffness is essential, as the causes, clinical course, and treatment differ. Primary shoulder stiffness is a predominantly self-limiting inflammatory-fibrotic disorder of the joint capsule with a typical course consisting of a painful phase, a stiffness phase, and a recovery phase. In contrast, secondary shoulder stiffness develops following trauma, surgery, fractures, rotator cuff repairs, shoulder stabilization procedures, calcific tendinitis, etc. In addition to capsular contractures, extracapsular adhesions are frequently present in the subacromial, subdeltoid, and subcoracoid spaces. While primary shoulder stiffness often resolves with conservative treatment, the results in secondary forms are frequently limited. In cases of persistent, functionally relevant stiffness, arthroscopic arthrolysis represents a successful surgical treatment option. During this procedure, the contracted parts of the joint capsule, including the rotator interval as well as the anterior, inferior, and, if necessary, posterior capsule, are released intra-articularly. In addition, secondary forms frequently require extra-articular arthrolysis with release of adhesions in the subacromial, subdeltoid, and subcoracoid spaces in order to restore the physiological gliding capacity of the soft tissues. Concomitant surgical procedures are also important, such as rotator cuff reconstruction, implant removal, excision of calcific deposits, and treatment of biceps tendon pathologies. Numerous studies have demonstrated very good improvements in pain, range of motion, and shoulder function following arthroscopic arthrolysis. Many patients regain nearly normal function in daily activities and are able to return to overhead and throwing sports. Consistent physiotherapy following arthroscopic arthrolysis is essential.
