Hyaluronsäure intraartikulär bei Gonarthrose*
Zusammenfassung: Katamnestische Auswertung von 170 symptomatischen Gonarthrosepatienten der Jahre 1994–2014, die im Zuge einer konservativen Therapie insgesamt 317 intraartikuläre Injektionsserien mit Hyaluronsäurepräparaten (216 behandelte Kniegelenke) erhielten. Ein gutes Ergebnis mit weitgehender Beschwerdefreiheit über zumindest 6 Monate wurde in 67,8% der Fälle erreicht, in 12,3% wurde ein schlechtes Resultat verzeichnet. Wesentliche Komplikationen wurden nicht beobachtet. In 19 Fällen (8,8%) war bei Abschluss der Studie (2016) zwischenzeitlich ein alloplastischer Gelenkersatz durchgeführt worden.
Das Behandlungsergebnis korrelierte mit dem Lebensalter, dem Arthrosegrad des betroffenen Kniegelenks sowie dem Body-Mass-Index, nicht jedoch mit dem Geschlecht. Patienten mit überwiegend femoropatellaren Veränderungen waren zufriedener als solche mit femorotibialen Knorpeldegenerationen.
Es bleibt zu folgern, dass eine intraartikuläre Hyaluronsäure-Applikation im Falle einer Gonarthrose durchaus einen akzeptablen Stellenwert in der konservativen Behandlung besitzt.
Summary: Catamnestic examination of 170 patients with symptomatic arthritis of the knee joint (1994–2014), who received 317 intraarticular injection series of hyaluronic acid (216 treated knee joints). A good result (free of pain for at least 6 months) was achieved in 67.8%, 12.3% showed a poor result. There were no severe complications. At the end of the study (2016) an alloplastic joint replacement of the knee had been performed in 19 cases (8.8% of the treated knees). We could prove a correlation of the result with the age of the patient, the stage of the arthritis as well as the body-mass-index, not with the sex. Patients with mostly femoropatellar changes showed more satisfactory results than patients with femorotibial joint destructions. All in all we may conclude, that the intraarticular application of hyaluronic acid in case of patients with symptomatic knee arthritis may be recommended.
