Zusammenfassung: Die femoropatellare Instabilität gilt als prädisponierender Faktor einer femuropatellaren Arthrose und ist multifaktoriell bedingt. Die entsprechenden Elemente umfassen die Muskulatur, kapsulo-ligamentäre und knöcherne Strukturen. Dieser Artikel behandelt insbesondere die knöchernen Faktoren, genauer genommen die Dysplasie der Patella und der Trochlea. Radiologische Klassifikationen dienen hierbei primär der Bewertung der einzelnen Risikofaktoren der femuropatellaren Instabilität. Darüber hinaus trägt die radiologische Bildgebung zur Charakterisierung von zugrunde liegenden anatomischen Anomalien, Patellafehlstellungen und möglichen Weichteilverletzungen bei. Die Ergebnisse dieser Untersuchungen können orthopädischen Chirurgen bei der Therapieentscheidung und dem post-operativen Monitoring unterstützen.
Summary: Patellofemoral instability is multifactorial and considered to be a predisposing factor for patellofemoral arthrosis. The corresponding elements include the musculature, capsulo-ligamentous and bony structures. This article deals in particular with the bony factors, more precisely with dysplasia of the patella and trochlea. Radiological classifications are primarily used to assess the individual risk factors of femuropatellar instability. Furthermore, radiologic imaging contributes to the characterization of underlying anatomic abnormalities, patellar malalignment, and possible soft-tissue injuries. The results of these examinations can aid orthopedic surgeons in decision making when to operate and to monitor patients postoperatively.
