Die Behandlung der Facettengelenk- arthrose mit Hyaluronsäure
Zusammenfassung: Das sog. Facettensyndrom ist ein chronisches Schmerzsyndrom aufgrund einer Reizung der in der Gelenkkapsel liegenden Nozizeptoren, hervorgerufen durch chronische Überlastungen, degenerative Bandscheibenschäden oder Arthrosen. In einer offenen, prospektiven, monozentrischen Studie wurden 80 ambulante konsekutive Patienten im Alter von 36 bis 89 Jahren mit chronischer Facettengelenkarthrose mit dem Hyaluronsäurepräparat (HA) Curavisc mini (1ml Fertigspritze zu 10mg durch Fermentation gewonnene HA) behandelt (3 Injektionen in wöchentlichem Abstand). Nach der jeweils letzten Injektion wurde die Wirksamkeit anhand Visueller Analogskalen (VAS) mit einer Skalierung von 0–10 bewertet. Ausgewertet werden konnten 79 Patienten, für die nach 3 Injektionen (n=74 Pat.), resp. 2 Injektionen (n=5 Patienten) alle Angaben komplett vorlagen. Die Schmerz- und Beschwerdesymptomatik nahm während des 3-wöchigen Behandlungszyklus bei allen Kriterien kontinuierlich ab. Die intraartikuläre Injektionstherapie mit dem gut zu handhabenden HA-Präparat Curavisc mini scheint eine den Patienten wenig belastende und nachhaltige Alternative in der Behandlung des chronischen Facettensyndroms zu sein.
Summary: The facet syndrome is a chronic pain syndrome caused by an irritation of the nociceptors in the joint capsule. Major causes are chronic overloading, intervertebral disc degenerations or osteoarthritis. In an open, prospective monocentre study 80 consecutive out-patients between 36 and 89 years with chronic facet joint arthropathy were treated by 3 intraarticular injections with the hyaluronic acid (HA) product Curavisc mini (1ml pre-filled syringe à 10mg hyaluronic acid gained by fermentation) in weekly intervals. After the last injection the efficacy of the treatment was evaluated with visual analog scales (VAS) with a scaling from 0–10. 79 patients could be evaluated after 3 injections (n=74 pat.), or after 2 injections resp. (n=5 pat.) where all data were complete. All criteria of pain and complaints continuously improved during the 3 weeks’ course of the treatment. The intra-articular injection treatment with hyaluronic acid seems to be an uncomplicated, clinically effective and enduring alternative to systemic or topical anti-inflammatories or antirheumatics in the treatment of the chronic facet syndrome.
