Zusammenfassung: Die Skaphoidfraktur als häufigste Fraktur der Handwurzel stellt noch heute ein Problem in Diagnostik und Therapie dar. Auf die aktuell notwendige frühestmögliche Diagnostik wird hingewiesen. Zudem werden Entscheidungshilfen für die Therapieauswahl mit der gebräuchlichsten Frakturklassifikation nach Herbert und Krimmer aufgezeigt. Die konservative Therapie der Skaphoidfraktur wird beschrieben und die etablierten operativen Verfahren werden dargestellt. Die doppelgewindige kanülierte Herbert-Schraube ermöglicht die derzeit optimale operative Versorgung.
Abstract: The scaphoid fracture is the most frequent fracture of the carpus and still poses a diagnostic and therapeutic challenge. The currently required method for earliest possible diagnosis is outlined. We offer guidelines for decisions regarding therapy based on the fracture classification according to Herbert and Krimmer. We describe the conservative therapy for the scaphoid fracture as well as the established operative procedures. The double-thread Herbert-screw actually provides the optimal surgical treatment.
