Das rheumatische Schultergelenk
Zusammenfassung: Im Verlauf der rheumatischen Grunderkrankung kommt es regelhaft zu einer Mitbeteiligung des Schultergelenkes. Dies wird in der subjektiven Wahrnehmung des Patienten häufig vernachlässigt, da Symptome an anderen Gelenken im Vordergrund stehen. Der fortlaufende rheumatische Entzündungsprozess intra- und periartikulär führt zu einer progredienten Destruktion des Gelenkes und der Rotatorenmanschette. Nach Ausreizen der konservativen Therapie mit systemischer medikamentöser Basistherapie, Physio- und Ergotherapie, lokal symptomatischer Therapie und intraartikulärer Glukokortikoidinjektion sollte frühzeitig die operative Therapie in Erwägung gezogen werden. Insbesondere in frühen Stadien (LDE 0–3) kann durch Synovektomie und Versorgung von Begleitpathologien der Entzündungsprozess aufgehalten und die Beschwerdesymptomatik verbessert werden. Bei fortlaufender Krankheitsaktivität sollte mit der endoprothetischen Versorgung nicht zu lange gewartet werden, weil die progrediente Destruktion insbesondere der Rotatorenmanschette eine Alloarthroplastik erschwert und zu schlechten Ergebnissen führt.
Abstract: During the course of rheumatic disease the shoulder joint is frequently involved. Since the symptoms of other joints are in focus, the shoulder is often neglected by the patients. The current intra- and periarticular rheumatic inflammation process causes the progressive destruction of the joint and the rotator cuff. After insufficient conservative therapy combining basic therapeutic drugs, physio- and ergo therapy, local symptomatic therapy and intra articular glucocorticoidinjection, the surgical therapy should be considered in early stages of destruction (LDE 0–3). Especially in early stages the inflammatory process can be reduced by synovectomy and surgical management of additional pathologies. In case of persisting disease activity the progressive destruction of the rotator cuff could complicate an alloarthroplasty, therefore it should be considered in an earlier stage of disease.
