Aktuelle Therapiekonzepte bei Knorpelschäden am Talus
Zusammenfassung: Die operative Behandlung von Knorpelschäden am Talus des oberen Sprunggelenks bietet eine Vielzahl von Techniken. Für die Auswahl des geeigneten Verfahrens ist die Kenntnis von Ätiologie und Art des Knorpelschadens von großer Bedeutung. Physiologisch und biomechanisch unterscheidet sich der Talus wesentlich vom Kniegelenkknorpel. Daher müssen hier andere Kriterien der Knorpelreparatur zugrunde gelegt werden. Ziel dieser Übersichtsarbeit ist es darzustellen, welche regenerativen, Verfahren zum Aufbau von chondralen und osteochondralen Defekten zum Einsatz kommen können.
Summary: The surgical treatment of cartilage lesions of the talus offers a variety of techniques. For choosing the appropriate technique, knowledge about etiology and type of the defect is essential. Physiologically and biomechanically talus cartilage differs essentially from knee cartilage. Therefore different criteria for cartilage repair must be underplayed. Aim of this publication is to outline current regenerative procedures in chondral and osteochondral defects of the talus.
Gelenkerhaltende operative Maßnahmen bei der Gonarthrose
Zusammenfassung: Die operative Behandlung der Gonarthrose – bevor ein endoprothetischer Gelenkersatz erforderlich ist – bietet eine Vielzahl besonders arthroskopischer Techniken. Diese gelenkerhaltenden Operationstechniken können, dank verbesserter Bildgebung, schon in frühen Formen der Gonarthrose und bei präarthrotischen Veränderungen stadienadaptiert erfolgreich eingesetzt werden. Ziel dieser Übersichtsarbeit ist es, darzustellen, welche Therapieformen von der resezierenden (Debridement, Meniskuschirurgie, Osteophyten-Resektion, Synovektomie), reparativen (Bandplastiken), oder regenerativen Chirurgie (Knochenmarkstimulation, Chondrozytentransplantation) bis zu Umstellungsosteotomien zum Einsatz kommen können. Alle genannten operativen Verfahren sind erfolgreich anwendbar, sofern die entsprechende Pathologie adressiert und die Indikation richtig gestellt wird. Die Gonarthrose selbst ist durch keines der Verfahren reversibel. Auch die individuelle Progredienz der Arthrose beeinflusst das Behandlungsergebnis entscheidend.
Abstract: The surgical treatment of osteoarthritis of the knee – before an endoprosthetic joint replacement is required – offers a variety of techniques. Thanks to improved imaging, these joint-preserving techniques can be applied in early stages of osteoarthritis or pre-arthritic changes. The aim of this review is to present therapy forms like resection (debridement, meniscus surgery, osteophyte resection, synovectomy) repair (ligament reconstruction), regeneration (bone marrow stimulation, chondrocytes) and realignment (osteo-tomies). All of the above surgical procedures are successfully used if the corresponding pathology is addressed well, and the indication is set properly. The gonarthrosis itself cannot be reversed by any of the methods. Also the individual progression of osteoarthritis affects treatment outcome significantly.
