Zusammenfassung: Die arthroskopische anatomische Gelenkrekonstruktion mittels autologer Beckenkammplastik stellt eine verlässliche Behandlungsmethode bei der anteroinferioren Schulterinstabilität mit knöchernem Glenoidranddefekt dar. Neben potenziellen kosmetischen Vorteilen besteht hier insbesondere die Möglichkeit des Integritäterhalts der Subscapularissehne mit Vermeidung
einer sekundären Verfettung und Atrophie. Es wurden sehr gute klinische und radiologische Ergebnisse mit einem überschaubaren Komplikationsspektrum beobachtet, wobei sich computertomografisch eine anatomische Wiederherstellung der Konkavität des Glenoids zeigte.
Summary: Arthroscopic anatomic glenoid reconstruction using an autologous iliac-crest bone graft represents a reliable technique for the treatment of anteroinferior shoulder instability with bony glenoid rim defects. In addition to potential cosmetic benefits, the preservation of the integrity of the subscapularis tendon constitutes the main advantage. A fatty degeneration and atrophy of the subscapularis muscle can therefore be prevented. Very good to excellent clinical and radiological results were observed with only few complications. Computed tomographic imaging showed an anatomic reconstruction of the glenoid concavity.

Zusammenfassung: Die superiore Kapselrekonstruktion (SCR) stellt einen innovativen Behandlungsansatz bei irreparabler posterosuperiorer Rotatorenmanschettenruptur dar. Ziel ist es, einer vertikalen Translation des Humeruskopfs entgegenzuwirken und die superiore Stabilität des Glenohumeralgelenks wiederherzustellen. Der vorliegende Artikel gibt einen Überblick zur Indikationsstellung, der operativen Technik sowie den ersten klinischen und radiologischen Ergebnissen dieses innovativen Verfahrens.
Summary: The superior capsular reconstruction (SCR) has been developped for the treatment of irreparable posterosuperior rotator cuff tendon tears. The aim is to prevent vertical migration of the humeral head and re-establish superior stability of the glenohumeral joint. This article aims at giving an overview of the indication, surgical technique and current clinical and radiological results of this innovative procedure.
