Hallux rigidus
Zusammenfassung: Nach dem Hallux valgus ist der Hallux rigidus eine der häufigsten Erkrankungen des Großzehengrundgelenks und die häufigste Arthrose des Fußes, die zuletzt in der fortgeschrittenen Arthrose des Großzehengrundgelenks endet. Die zur Erkrankung führenden Ursachen sind zum jetzigen Zeitpunkt nicht geklärt, es werden jedoch verschiedene prädisponierende Faktoren diskutiert. Die klinische Symptomatik präsentiert sich in Abhängigkeit von dem vorliegenden Stadium als lokaler Schmerz, Bewegungseinschränkung, Schuhkonflikt und zuletzt kompletter Einsteifung des Gelenks. In der Therapie kommen gelenkerhaltende ebenso wie das Gelenk opfernde Verfahren zur Anwendung, welche unter Bezug sowohl auf das Krankheitsstadium als aber auch auf individuelle Patientenfaktoren sorgfältig ausgewählt werden müssen.
Summary: Hallux rigidus is one of the most common diseases of the first metatarsophalangeal joint and the most common arthritis of the foot that recently ends in severe osteoarthritis of the joint. The cause of the hallux rigidus is still not determined, although several predisposing factors have been cited. Depending on the degree of degeneration different symptoms can be found such as local pain, limitation of movement, shoe conflict and finally complete stiffness of the joint can occur. Surgical treatment can basically be joint preserving or joint sacrificing depending on the progress of the degenerative process. The decision making is based on the grade of osteoarthritis, joint mobility and individual aspects of each patient. A very precise clarification has to be done referring to patient’s needs and expectations.
